Telehealth Consent | AgeWell Direct
Telehealth is the delivery of healthcare services using secure electronic communication rather than an in-person visit. The consent you sign during the secure evaluation is provided by Independent licensed provider network; this summary is informational.
What telehealth involves
A licensed provider reviews the information you submit, and may communicate with you by secure message, telephone or video. The provider may request laboratory testing or additional records before making a determination.
Benefits
Convenient access to a licensed provider, reduced travel, and documented clinical review of whether therapy is appropriate for you.
Limitations and risks
Telehealth cannot replace an in-person physical examination in all circumstances. Information you provide may be incomplete, technology may fail, and the provider may determine that in-person care is required. Delays in evaluation or treatment are possible.
Your agreement
You agree to provide accurate and complete information, to follow monitoring instructions, and to seek emergency care locally for urgent symptoms. You understand that the provider may decline to prescribe and that participation is voluntary; you may withdraw consent at any time.
Emergencies
Telehealth is not for emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
